Care Coordination
What is Care Coordination at Three Shires Medical Practice?
Care Coordination
A Care Coordinator works in a doctor’s surgery to help patients understand their health and get the care they need. They make sure all the different people helping the patient — like doctors, nurses, and other services — work well together. They also help patients learn how to manage their health and connect them with support groups or organisations that can help.
At the surgery we have three Care Coordinators:
Sarah: Team lead. She invites patients to come in for yearly reviews, especially those with long-term illnesses like asthma, high blood pressure and diabetes. She also looks after cancer care pathways, NHS Health Checks and special clinics. Alongside this she also supports patients, and the rest of the team.
Victoria: Helps people who look after family members or friends (called carers). She can refer them to a monthly Carers Surgery and make sure they are on the Practice’s Carers Register. She also looks after our housebound patients, older patients that may be lonely or isolated and looks after our cervical screening processes.
Gemma: Looks after our patients that have dementia, learning disabilities or a severe mental illness. She ensures they are invited for their annual review and that they have any support they may need.
If you would like to speak to a care coordinator, please let the reception team at your surgery know and they can put you in contact.
