Care Coordinator Vacancy - 16 hours per week

We are seeking a Care Coordinator to join our team at The Three Shires Medical Practice, working as part of our Primary Care Network (PCN) multidisciplinary healthcare team. This role is central to improving patient care and supporting individuals across health and social care services.

The post-holder will provide coordination and navigation for patients and their carers, ensuring they receive the right support at the right time. You will work closely with GPs, practice teams, social prescribing link workers, and other primary care professionals to manage a caseload of identified and often vulnerable patients.

Key Responsibilities

  • Providing support to unpaid carers, helping them access resources and services to maintain their own wellbeing.
  • Supporting patients on the Severe Mental Health Register and Learning Disability Register, ensuring timely reviews and appropriate follow-up.
  • Assisting the Lead Care Coordinator with patient invitation workflows, helping to streamline processes and improve engagement.

  • Ability to work confidentily and independently in a varied and sometimes challenging environment.
  • Stong organisational skills to manage a caseload effectively.
  • Commitment to working with people from diverse cultural and social backgrounds.
