Long-Term Condition Yearly Review Information

 

This section explains our long-term health conditions yearly reviews. This is so you know what you can expect. 

 

We aim to review all long-term health conditions around your birth month. We will let you know when you can to book your appointment. This will be through a text or letter. 

 

You may need to see more than one person such as:

Doctor / Nurse / Health Care Assistant / Practice Pharmacist.

 

We will combine appointments where we can.

 

What health conditions are included:

 

Diabetes

Regular tests of diabetes are important. You will first need to attend the surgery for some tests. Please bring an early morning urine sample. This will be followed by an appointment with a nurse.

 

Heart Failure

You will first have an appointment for a blood test and blood pressure check. This will be followed by an appointment with a doctor.

 

COPD

We will send you a text message with a link for you to do a series of questions online. We will then check these results and contact you to arrange an appointment. If you don’t have a mobile, we will send you a letter.

 

Asthma

If you have Asthma and use inhalers, we will send you a text message with a link for you to do a series of questions online. We will then check these results and contact you to arrange an appointment. If you don’t have a mobile, we will send you a letter.

Chronic Kidney Disease (please bring an early morning urine sample)

Chronic Heart Disease / Stroke / TIA

High Blood Pressure / Pre-Diabetes

You will need a 20-minute appointment for a blood test and blood pressure check.

 

Dementia

Learning Disability

Specific Mental Health Conditions Schizophrenia / Bipolar Disorder / Psychosis

We have a Care Coordinator Team that will help you book your yearly reviews. They will be in contact with you directly.
