If you're planning to travel outside the UK, you may need to be vaccinated against some of the serious diseases found in other parts of the world.
As you will be aware, GP surgeries are working under increasing pressure and must prioritise the services they provide.
Therefore, the basic travel vaccinations funded by the NHS are available at your surgery - provided you follow the guidance in the Travellers Information Sheet.
Please be aware we no longer offer private travel vaccinations, malaria tablets or a full travel assessment service.
If you wish to have any of the vaccinations which are provided on the NHS, please dowload a Three Shires Travel Risk Assessment Form, complete it and leave it with your surgery reception.
It is your responsibility to ascertain which vaccinations you need, and, to inform yourself fully of recommended travel safe precautions.

Some countries require proof of vaccination (for example, for polio or yellow fever vaccination), which must be documented on an International Certificate of Vaccination or Prophylaxis (ICVP) before you enter or when you leave a country.
